Privacy CONSENT FORM for Residents of the Philippines:

• I hereby authorize Envision Learn, Inc., corporation duly registered in Delaware (the “Corporation”) and agree:

• To collect and process my personal data relative to my enrolment with its programs and courses.

• That my rights and the Corporation’s obligations relative to my enrolment and registration with the Corporation shall be defined and governed by American law.

• That, in case of conflict between American law and the laws of other jurisdictions with respect to said rights and obligations under paragraph 2 above, American law shall prevail.

• To waive all other reliefs and remedies available to me under the laws of Philippine laws in respect to the collection, processing and use of my personal data;

• I agree that any disputes arising from the activity described in paragraph 1 above shall be submitted before the appropriate American courts or tribunals, to the exclusion of all other courts or tribunals.

• I am represented herein by my guardian. They consent on my behalf to paragraphs 1 to 5 above with respect to my personal data. They likewise consent to paragraphs 1 to 5 above with respect to their personal data that will be provided in connection with my enrolment with the Corporation’s programs and courses.
